PHP Server Monitor is an open source platform for monitoring and managing your services and websites. When the monitored service and website are down, it will send you Email, SMS and Pushover notifications, and will log the errors and the messages. A cron job has been set up for automatically updating the status every 15 minutes. Additionally, the platform provides user management that you can manage users for each server with a mobile number and email address.
