Overcomplicated setup
There are many steps to getting this app to work and the instructions are spread across three locations: 1. apps.splunk.com has documentation to get started. 2. But the app doesn't work with the version of splunk installed on the ami, so you have to go to docs.splunk.com to read how to perform an upgrade. 3. Instructions for connecting the app to your AWS environment are in the app itself once it's running.
I spent several hours trying to get the app to report on our data before ultimately giving up.

Works well, but is not perfect yet.
I use this AMI in combination with Hunk App for AWS Elastic Load Balancing (http://apps.splunk.com/app/1731/?ref_=7). To make that work you need to get a more recent Splunk version installed over version 6.0 on this AMI.
Splunk support was very helpful and responsive.
