Agentic AI Where Connectivity Is Scarce: MedZen Health in Cameroon

Different expertise, one shared purpose
In rural Cameroon where connectivity is limited, seeing a doctor can mean losing a day’s wages, traveling for hours on unpaved roads, and arriving at a clinic with no medical history on file. For many people, these barriers prevent care altogether.
Alain Bagmi and Cynthia Achu of MylesTech Solutions, created MedZen Health after losing loved ones in this fragmented healthcare system. They believed timely access to quality healthcare could prevent future tragedies. Their shared loss gave them a common purpose, while their different professional backgrounds shaped how they responded.
Alain’s experience taught him that technology had to work beyond major cities and in areas with limited infrastructure. “I wanted to build something for people in rural communities, for single mothers traveling hours to clinics, and for farmers who cannot afford to lose a day’s wages to see a doctor,” says Alain.
Cynthia’s experience led her to understand that the patient needs to be at the center of any healthcare technology. “Healthcare is deeply personal,” Cynthia says. “We knew this had to be built around people’s real lives, not just their medical needs, but their time, their resources, and their trust.”
Together, Cynthia and Alain designed MedZen Health around the conditions many Central African communities face: basic mobile phones, limited connectivity, long travel distances, disconnected medical records, and healthcare facilities without shared information systems.
Meeting Patients on Any Phone, Any Connection
One challenge with connecting patients to clinical facilities is the lack of smartphones and broadband. Approximately 70% of Cameroon’s healthcare costs are paid out of pocket by households, leaving many families unable to afford basic consultations. Mobile connectivity costs consume over 13% of per-capita income for those who can access it. Tens of millions of mobile phones in active use across the region are feature phones, meaning millions of potential patients have no way to download healthcare apps.
To address this, MedZen Health supports SMS, Unstructured Supplementary Service Data (USSD), and call center access, helping patients connect with healthcare services regardless of their phone type or internet access.
Agentic AI Built for Connectivity Constraints
At the core of MedZen Health is an agentic AI architecture. It uses 15 specialized AI agents powered by Amazon Bedrock AgentCore, with three to five agents working together during each patient interaction. A central orchestrator coordinates these agents and assigns each task based on the patient’s needs. One agent may gather symptoms, turn them into structured data, and prepare a summary for the clinician. Others help patients find a provider, schedule appointments, or document consultations. Each agent only accesses the information and tools required for its assigned role.
Additional agents support the journey when needed, helping with laboratory results, medication availability, medical documents, retinal images, and updates to the patient’s health record. After the consultation, a follow-up agent contacts the patient within 48 hours, records the response, and alerts the provider if a concern is identified. Together, these agents connect previously fragmented stages of care, while secure backend controls govern access to patient information and qualified healthcare professionals retain responsibility for diagnoses, prescriptions, and treatment decisions.
The platform runs offline-first. Patients in remote villages can record symptoms, access health information, and view medical history without internet. When connectivity returns, the platform automatically synchronizes with cloud infrastructure. No data is lost, and privacy is preserved through HIPAA-eligible audit trails.
For patients with limited literacy or those who prefer voice-based communication, MedZen Health uses Amazon Nova Sonic to provide natural, real-time conversational interactions. Amazon Polly supports spoken prompts and responses, while Amazon Comprehend Medical helps identify and structure relevant clinical information from patient conversations. Together, these services allow MedZen Health’s agents to adjust how information is presented, making healthcare interactions easier to understand and navigate.
The platform’s “text-first” design means that most consultations are resolved entirely through secure messaging: a physician reads a symptom description, reviews photos and vital signs, provides diagnoses, and issues prescriptions, all without requiring high-speed internet or video calls.
For the millions who rely on basic feature phones, MedZen Health is developing USSD integration, the same technology that powers mobile money transfers. USSD allows patients to dial a number, navigate text menus, describe symptoms, and receive guidance with no internet and no smartphone required.
Making an Impact, One Healthcare Facility and Patient at a Time
Scaling Care Without Infrastructure
Doh’s Medical Center serves patients from three surrounding regions. Before MedZen Health, patients traveled for hours for a single consultation, many of whom could have been triaged remotely. With the MedZen Health platform, the center now manages 120 active patients through a combination of in-person and remote consultations. The AI appointment system handles scheduling without dedicated administrative staff. Patients receive reminders automatically. Prescriptions are digitally issued to partner pharmacies. The result: reduced patient travel burden, better retention in care, and more efficient use of limited staff.
Reaching Beyond the City
Dr. Sammy Orock Oben at WeCCARE Foundation recognized that telemedicine could extend the clinic’s reach into rural villages that would otherwise have no specialist access. Using MedZen Health, Dr. Oben consults patients from surrounding villages without them traveling to the hospital. “A woman with a urinary tract infection does not lose a day’s wages traveling to the city anymore,” Dr. Oben explains. “She gets diagnosed and treated within hours, all from her phone, using minimal data.”
Transforming the Patient Experience
“For a long time, I could not find a healthcare facility in Douala that offered the mammography I needed,” recalls Laura Pearl Mah. “I visited several clinics and laboratories, only to learn that the doctor was unavailable, the equipment was not working, or the examination was not offered. I spent time and money going from place to place with little information to guide me.”
Then a friend introduced Laura to MedZen Health. Using MedZen Health, Laura found a medical imaging center that provided mammography and booked her visit online.
Shortly after her visit, Laura received a text message referring her for follow-up mammography testing. She again used MedZen Health to arrange the visit and receive those follow-up results through the secure portal, in just 2 days.
“Before MedZen Health,” Laura said, “seeking medical care often meant wasted journeys, uncertainty, and stress. MedZen Health gave me a simpler way to find the right place, plan my visits, follow the next steps, and receive my report. MedZen Health is now the first place I turn to when I need care, and I recommend it to everyone I know.”
Strengthening Blood Access Across Africa
Access to specialist care is only one dimension of the challenge. When patients need emergency transfusions, fragmented donor coordination and the absence of real-time information create life-threatening delays. LifeLine Africa is strengthening blood access systems by building the community and digital infrastructure that connects voluntary donors, hospitals, and technology.
Through its integration with MedZen Health, Lifeline can manage and grow its donor network, coordinate emergency blood requests, automate donor notifications, and collaborate with healthcare providers using MedZen’s secure, offline-first platform.

Early Detection for Breast Cancer
In many African countries, breast cancer is diagnosed at late stages because women lack access to screening, education about warning signs, and affordable treatment. Dare To Live bridges this gap by mobilizing community health workers to conduct screenings and raise awareness in underserved areas.
When Dare To Live’s community health workers identify women who might have breast cancer, MedZen Health becomes the bridge to specialist care. Women can connect with oncologists without traveling long distances. When patients need imaging, MedZen Health coordinates scheduling, arranges specialist video consultations to review results, and issues treatment recommendations.
“Early detection saves lives, but only if the woman who detects a lump can actually see a doctor,” says Akwi Tah Anjoh, Founder of Dare to Live. “Now, in many cases, a woman in a village 100 kilometers from the nearest oncologist can get expert guidance within 24 hours from her phone. This is how we reduce breast cancer deaths: by making early detection matter.”
Technology as Mission: Building on the Right Foundation
MedZen Health is built on AWS cloud infrastructure, using agentic AI developed using cloud-native technologies and AWS’s global network to deliver low-latency, reliable service across Central Africa.
“Technology is the vehicle,” said Alain. “The mission is to ensure that a mother in Edea gets the same access to specialist care as someone in Yaounde. That a health worker in a rural clinic can manage dozens of patients with confidence. That a mother in labor does not die because blood was not available. And that a woman discovers a breast lump and can reach an oncologist before it becomes too late.”
