You pay by the hour based on the EC2 instance type you choose to run Moodle. The 21 options map to different AWS instance families and sizes, not software feature tiers. Each unit is one running EC2 instance measured per hour. You pick one instance type, such as t3.medium or r5.large, and pay the hourly rate for as long as that instance runs. The type sets its compute and memory capacity, not any software feature difference.
The software rate meters running hours only. A fully stopped instance stops accruing the hourly software charge. Underlying AWS charges, such as storage for the attached volume, may still apply while the instance is stopped. Charges resume when you restart the instance.
The hourly rate covers the Moodle software and vendor support included with the listing. AWS infrastructure charges, such as compute, storage, and data transfer, are billed separately by AWS. Your total combines the software rate for your chosen instance type plus these AWS charges.
