This AI-powered care planning platform streamlines how care providers create, manage, and update personalised care plans. By leveraging Amazon Bedrock, it transforms client-provided documentation into structured, high-quality care plans in seconds. Care partners can easily search for clients, select relevant documents, and generate plans through an intuitive React-based interface, reducing manual effort and improving consistency across care delivery. The solution was recognised as a runner-up in the 2025 APAC Gartner “Eye on Innovation” award, highlighting its impact and innovation in the care sector.

The platform integrates seamlessly with care management platforms such as AlayaCare to automatically sync client records, forms, and care plans, ensuring systems stay aligned without duplicate data entry. It is designed with a flexible integration layer, meaning it can be connected to other care management systems as needed. Real-time communication via REST and WebSocket APIs, combined with event-driven updates through AWS SQS, ensures that new form submissions and changes are reflected immediately.

The results are significant: care plan generation time has been reduced from an average of 20 minutes to just 32 seconds, while the overall care planning process has decreased from around 60 minutes to 40 minutes. This delivers faster turnaround times, improved accuracy through AI-assisted generation, and greater efficiency for care teams - allowing them to focus more on delivering high-quality, personalised care.
