Overview
This dataset comprises 45 unique-patient inpatient clinical note records, sourced from a single U.S. institution. Notes are drawn from a single hospitalization per patient, with the majority of encounters spanning less than 30 days.
Each patient includes a minimum of 15 clinical notes, spanning a variety of note types generated throughout the inpatient care journey: progress notes, physician specialist consults, physical/occupational/speech therapy notes, and case management/social worker notes. Every patient includes an admission note, documenting the start of the inpatient encounter. All notes include a documented Assessment/Plan or equivalent structured section reflecting clinical decision-making and care planning.
Patients are represented across 10 clinical specialties/care pathways: Cardiology, Infectious Disease, Hematology/Oncology, Pulmonology, Geriatrics, General Medicine, Endocrinology, GI, Orthopedics, and Neurology. Level of care is documented across General Floors, ICU, and Step Down Unit settings.
The cohort includes adult patients only (ages 25–90), with race representation, balanced by sex (58% male, 42% female). All notes are in English and in finalized status.
