Why OpenMRS, and why this image
OpenMRS powers patient records for health systems in more than forty countries and underpins some of the largest HIV, TB and primary care programs in global health. It is modular, standards aware (FHIR support via module) and backed by a large implementer community. What it is not is simple to stand up: the platform, frontend, Tomcat, Java and database each have version interdependencies that consume implementer time better spent on clinical configuration. This image arrives assembled, secured and documented, so your first day is spent on forms and workflows, not dependency resolution.

Consider using encrypted volumes, network isolation, access control, audit trails, backup handling.
Place instances in private subnets where clinical workflows allow, restrict the web tier to your networks, and never expose the database port.
Sizing guidance
Evaluation and training: t3.large (Java needs memory; smaller instances will disappoint). Single facility production: m7i.large with encrypted gp3 storage. Multi site or heavy reporting: m7i.xlarge and consider RDS for MariaDB, documented in the architecture guide.

You pay by the hour for the AWS EC2 instance that runs this pre-packaged OpenMRS image. Pricing is tied to the instance type you choose, so it scales with the compute, memory, and storage of that machine. Smaller burstable types like t2, t3, and t3a cover light testing. General-purpose m4, m5, m6, and related families suit steady workloads. Memory-focused r5, r6, and related families fit larger data needs, up to bare-metal sizes. You run the same software across every option; only the underlying hardware and hourly rate change.
Top-of-mind questions for buyers
What does the hourly rate cover, and what does one instance include when I launch it?
The hourly rate covers the pre-packaged OpenMRS 3 image running on one EC2 instance. The database schema, modules, and demo content are imported at build time. Each instance rotates its own database and admin credentials on first boot, so first startup takes minutes rather than an hour.
Am I charged the software rate when the instance is stopped or powered off?
The hourly software rate meters running time only. A fully stopped instance stops accruing the software charge. You may still pay underlying AWS fees, such as storage for the attached volume, while the instance sits stopped. Restarting the instance resumes the hourly software charge.
If I need more compute later, do I switch instance types or upgrade a plan?
There is no plan to upgrade. You choose a different EC2 instance type when you launch or relaunch. The same OpenMRS software runs on every option; only the hardware size and the hourly rate change. Moving to a larger family like r5 or bare-metal raises the hourly rate accordingly.
