Why OpenEMR, and why this image
OpenEMR is the most widely deployed open source practice management and EHR platform, with two decades of development behind its clinical records, scheduling, billing workflows (including X12 claim generation), prescriptions, patient portal and a modern FHIR R4 API. It replaces per provider per month subscription EHRs with software you own and control. What stands between most practices and that outcome is assembly and security: PHP, web server, database and portal configuration, done correctly for patient data. This image arrives with that work complete and documented.

Choosing between OpenEMR and OpenMRS
Both are mature open source EMR platforms and CloudSOE publishes hardened images for both. Practices and clinics that need scheduling, insurance billing and a patient portal choose OpenEMR. Compliance: read this before storing patient data
An AMI cannot make a deployment compliant; operations do. This image gives you a hardened starting point, and the documentation is explicit about the remaining shared responsibilities:
Consider using encrypted volumes, network isolation, access control, audit trails, backup handling.
Place instances in private subnets where clinical workflows allow, restrict the web tier to your networks, and never expose the database port.
Restrict the staff interface to your practice networks or VPN. If you enable the patient portal, treat it as an internet facing application: it is the only component that should ever face the public internet, and the enablement guide covers its isolation.

You pay by the hour for the software running on your chosen Amazon EC2 instance. Every dimension maps to one EC2 instance type, so pricing scales with the compute you pick. Smaller shared-CPU types like t3.nano and t2.small support light workloads. Mid-range general-purpose families (m5, m6i, m7i, m7a) and their variants suit steady clinic use. Memory-optimized families (r5, r6i, r6a and related types) handle larger databases. The largest sizes, including 24xlarge, 48xlarge and metal options, serve heavy loads. Billing runs through your AWS account. You raise or lower cost by resizing instances.
Top-of-mind questions for buyers
What does one hourly unit cover, and what do I get for that price?
Each hourly unit is one running Amazon EC2 instance of the type you select. It runs the packaged OpenEMR software on a hardened Ubuntu base with Apache, PHP, MariaDB and HTTPS pre-configured. You pay the software rate for every hour that instance runs, per instance.
Am I charged the hourly software fee when my instance is stopped?
The software fee meters running time only, so a stopped instance stops accruing that charge. Underlying AWS resources like storage may still bill separately while the instance is stopped. Restarting the instance resumes the hourly software charge automatically.
How does my cost change if I move to a different EC2 instance type?
Each instance type maps to its own hourly rate, so resizing changes your rate immediately. Larger memory-optimized or metal types bill more per hour than small shared-CPU types. You control cost by choosing an instance sized to your clinic's workload and switching when needs change.
