I've had a good experience with FormAssembly
What do you like best about the product?
The user interface for adding and moving questions
What do you dislike about the product?
Sometimes the Salesforce connection can be confusing and dependent too much on formulas.
What problems is the product solving and how is that benefiting you?
Instead of asking for information in an email and having to compile myself, we use a form in FormAssembly and can wait to collect all info then manipulate in the way we'd like.
Designing Online Forms Can't Get Much Easier.
What do you like best about the product?
FormAssembly provides our agency with a flexible and easy tool to quickly roll out responsive forms. The intuitive design makes adding questions and enhancing the form with logic a quick task. For forms that fit into a longer process the ability to link FormAssembly to Salesforce through Connectors is a huge productivity enhancement by allowing users to skip Excel altogether.
What do you dislike about the product?
Managing form themes could be more intuitive. Our users complain that it is difficult to quickly create new forms that look modern or match existing branding. Background images can be difficult to use without some experience with html or css.
What problems is the product solving and how is that benefiting you?
FormAssembly has become a secure way to collect data from customers in our grant and incentive programs. It allows for our data to be moved into Salesforce and create contact, account, and application records that were entered by hand in days past. Our years of experience with FormAssembly allows us to use the platform in a variety of use cases with very few technical problems.
FormAssembly Assessment to Date
What do you like best about the product?
I like that it is user friendly from the point of designing the form and the creating the connectors with Salesforce. I also like that you can send forms back to users to edit or complete if they have forgotten their passwords.
What do you dislike about the product?
I do not like the responses view page because it makes it difficult to find older responses. I wish you could customize more of the emailed templates as well. Like the email verification reminder email template.
What problems is the product solving and how is that benefiting you?
We have implemented an online financing application using FormAssembly. Theis new form has allowed us to increase the number of submittals we are processing each month.
Excellent Form Tool w/ Salesforce Connectivity
What do you like best about the product?
The ease of building forms. The learning curve is not very steep and complex forms with multiple conditional rules don't become disorganized in the builder.
What do you dislike about the product?
Only negative would be that client-facing forms can have a basic look to them. The themes help, but it's difficult to build something "on brand".
What problems is the product solving and how is that benefiting you?
Internal organization has improved dramatically and documentation has less errors.
Recommendations to others considering the product:
They are investing in their support and documentation as well as the product.
Great Product
What do you like best about the product?
The Salesforce integration is excellent.
What do you dislike about the product?
The form-builder interface could be more user-friendly, and can slow down on complex forms.
What problems is the product solving and how is that benefiting you?
We've been able to automate a ton, by sending form data directly into Salesforce. Our event registrations zap cleanly to Salesforce, allowing us to report from there. Rules and repeating fields/sections let us build very complex forms that provide a great user experience.
FormAssembly makes it easy!
What do you like best about the product?
FA makes building forms easy and quick. It also allows users to build forms in the format of the anticipated medium - computer, tablet, mobile.
What do you dislike about the product?
The connectors, while OK, could be more user-friendly as it is when building the questions on a form.
What problems is the product solving and how is that benefiting you?
Event registration, query forms, applications. All are connected to other systems.

What do you like best about the product?
I really like the WSIWYG aspect and the conditional fields
What do you dislike about the product?
I dislike it when Form Assembly is unavailable.
What problems is the product solving and how is that benefiting you?
We are using it to process requests from students. They really appreciate the intelligent forms.
Excellent for Salesforce customers
What do you like best about the product?
Powerful two-way Salesforce integration and love the ability to include custom CSS and/or Javascript at the form level (vs it being global and affecting all forms).
What do you dislike about the product?
I would like to see better ability to prepopulate fields using a prefill connector (such as Salesforce).
What problems is the product solving and how is that benefiting you?
It has allowed us to capture leads and the ability to prefill forms with lead information from Salesforce help eliminate duplicate lead submissions for people looking to provide up-to-date information.
A great form tool built on the Salesforce platform
What do you like best about the product?
Great conditional logic in the integration - you can do so much matching with existing or new records!
What do you dislike about the product?
Don't have a ton of experience yet with styling the forms, but the user interface is a bit wonky.

Be prepared with how you will integrate with Salesforce in advance.

It is so easy to design and deploy surveys!!!

To be honest, we don't have any complaints.
What problems is the product solving and how is that benefiting you?
For Salesforce, we need a way to survey customers and easily bring that data back in our CRM. It is so easy to do this.
